I did some searching, and one person says:

I "hacked it" by simply including bad-behavior-generic.php from the
bootstrap.inc file. I would love to do it properly as a module,
incorporating logging and configuration but my php skills are minimal.
Since the modules are included from within a loop in a function, the
"global" variables aren't really global, and I haven't had time to
work out a way around this.
