Seems to me if authorized parties of all involved initial / sign off
on the change, you should be good. Perhaps better if uninvolved
witness signature too. [Do disabuse me if wrong.]
OTOH - could the bank be swallowing this because they know such is
irrelevant? (Assuming it is, here.) Which turns this into an ethical
issue - somebody watching you stroke something out, saying nothing,
and not informing you that they won't honour it.

Well yes, but that's true for anything, ain't it. If they don't like
the colour of your hair [irony!], they can choose not to do business
with you. Sometimes you have recourse, but not always. Assuming you
wish to put yourself through that effort. Charter of Rights not
withstanding?
As for hospitals ... agreements notwithstanding, a whole host of other
issues come into play too. Duress, informed consent, state of mind at
signing, and so on and so forth. And, after the fact, depth of desire
to dispute. Let alone, depth of pockets for $$$.
