What does the apache error log say? Check to see whether apache is trying to look 
for the file in /var/www or some other unexpected place. 

That's a wild guess. I am just learning this stuff myself. One thing I have 
learned is that the logs are really really important for debugging. I don't have 
a clue how people with restricted hosting debug these kinds of problems.
